FINAL REGISTRATION NIGHT

Items needed

*Completed Registration Packet
*Copy of Birth Certificate (new to program must bring original & copy)
*Wallet size photo - school photos are perfect
*Copy of front/back of health insurance card
*2 copies of final 2018-19 report card (if younger and report card isn't provided, parent must provide typed and signed note)
*Medical physical signed and dated between 8/1/18 - 7/31/19
*Payment in full or balance of payment

ALL ATHLETES MUST HAVE REGISTRATION ITEMS AND PAYMENT IN FULL IN ORDER TO BE ABLE TO TAKE THE FIELD ON AUGUST 1ST. NO EXCEPTIONS!
